International application

This is the full international admissions application to study in the United States on an F-1 student visa. It is long — set aside some time, and answer what you can. Fields marked * are required; everything else is optional and can be completed later.

Discovery Preparatory Academy is pursuing SEVP certification through the U.S. Department of Homeland Security. A U.S. school must be SEVP-certified before it can issue Form I-20, and it must first receive financial evidence covering the student’s educational and living costs. We accept applications now and issue Form I-20 to admitted students upon certification. Submitting this application does not guarantee admission or the issuance of any visa.

Has the student ever repeated or skipped a grade?

8. Recommendations

Provide contact details for each recommender. We’ll send them a recommendation form; your signature in section 14 authorizes us to request and review these records.

9. Conduct and disciplinary history
Has the student ever been suspended?
Has the student ever been expelled?
Has the student been placed on disciplinary probation?
Has the student been required to withdraw from a school?
Are any disciplinary matters currently pending?
Has the student had significant attendance problems?
Supporting documentation can be requested when applicable.
10. Health and student-support information

Basic information only. A detailed medical history, immunization record, and physical examination are handled on a separate health form after acceptance.

Can you provide proof of funds (bank statement or sponsorship letter)?

Optional now — you can also send these later. JPG, PNG, WebP, HEIC, or PDF, up to 6 MB each. Recommendation forms, immunization records, and certified translations are usually requested after acceptance.
